Whenever I use headphones, my phone randomly goes into voice dialing mode. It's annoying. It speeds up my music really fast, and even worse it's a complete drain on my battery. Even after I take the headphones out, the voice dialing icon is still visible on the top right-hand side of my screen. I've tried using the voice dialing options to turn it off, but there is no disable option. I don't even use voice dialing. Is there anyway for me to disable it when using headphones, or even better, just disable it completely?

There might be something in Options > Call Management where you can disable it. Also, the left convenience key might be mapped to Voice Dialing and is very easy to push by accident. I re-mapped mine to open Network Connections instead.

I've checked all over the options menu, as well as all of the other option menus, and couldn't find anything. And I remapped the left key a long time ago, but it still didn't help me. :(

Could be damage of some kind, water damage in particular. That will screw with your screen and make it more/less sensitive than normal, causing some apps to launch when you don't want them to. A drop could so something similar to this as well.

but I've never dropped it, and I certainly dont take it near water. I always keep it in my purse or my pocket whenever I need to wash my hands. Is there really nothing I can do to stop this from happening? I am seriously considering selling this phone and picking up another brand/model because of this issue.

Voice dialing is not automatically activated. It does require as Rambo stated a push of a key associated with the App. You might look at any interaction with an app. My guess is there is some interaction with the headset. Example the BlackBerry Premium
Multimedia Headset is capable of activating the VD by a simple push of a key even while playing music. Apparently your HS is doing this. You might try a different HS and see if it goes away( not the same model). The wiring configuration may duplicate this on some headsets.
