i have BES 4.03 and cannot add a specific user. i deleted him and tried to re-add and i get the err message "Failed to add user John Doe to blackberry Server. Unable to open mailbox for user. the users mailbox may have been deleted."

i am able to add other users just fine.
i have run iemstest.exe and it completes successfully.
when i run dbclean.exe on user workstation i get the message "nothing to Delete"
I have also double checked that the BES server and the Exchange server have the same exchange hotfixes and service packs applied. I'm at my wits end. please help.

Might be a silly question, but is the user a member of the exchange server that the MAPI profile is set up for (that might not be worded properly).

I am not able to add users to our BES in the US if their mailbox is on the Exchange server in Canada - they have to be on the Canada BES.

If there is more than one server involved, make sure BESAdmin has the appropriate rights to the server object where the user mailbox is homed. We had the same problem when we deployed a bunch out to users at remote sites where we'd never had Blackberry users before. Setting the rights/permissions to the Exchange home server for the BESAdmin account worked like a charm.
