Hi Superh3ro,
Just login into your account on T-Mobile website. Then you should see myFaves on bottom of list on R side and under that you should see manage my Favs. When you click on that will take you to page with your favs showing. On the L side you should see my Faves are missing. When you click that will then show Restore myFaves on R side. Click that and will send the Favs app to your phone. May take some time but will happen. I had to do the restore my Favs few times but eventually will get there.

Just to clarify-go to my.t-mobile.com then login there. If you are not registered then just register you phone number with a password.
