The service account gets permission to the accounts as added to the BES Server manager - not to the mail store or exchange, only to the mail accounts added.

The users status will stay on "Initializing" until you have completed the Enterprise Activation on the device.

Regarding the Mapi Error: In the Blackberry Server Configuration applet, have you clicked on "Edit Mapi Profile" and entered the correct server and mailbox name?

the service account is the one managing the software
the service account has full rights on AD and Echange
the services running on the server are managed by the service account
