Nothing really if you are on BIS.

For BES users it will reconcile mail.

It Reconciles the changes between the Server and Handheld prior to scheduled task for standard Automatic Reconciliations, Open, Mark Unopened, Deletions etc.

Yup. Email on the BB isn't ever synched. It's pushed and reconciled.
