This is a more "WTF?!" kind of thing, and I think I got it working, but if you ever try to deploy to a BB that doesn't have the phone application, and your application is a startup application, and it doesn't seem to work at all for some reason, try installing the phone application.

I was doing OTA installations to a device without the phone, and what clued me in was when someone clicked the "Run" button instead of "Ok", it had a crazy error about the phone application.

It is very strange, and probably a really rare edge case, since everyone deploys that by default. Of course, we use them for email and data only, so removing the phone made sense to me.

And my application has absolutely no references to Phone APIs whatsoever. Which makes me wonder WTF is going on. Seeing that the phone is generally a startup application as well, maybe clicking "Run" on a library cod file made it go crazy and make it miss the call to my libMain().

I'd contact RIM's tech support but they've been less than helpful so far. And I don't think the TSupport we have for BES covers development things or whatever. (Though, in the past, they've always tried replied, even when I didn't have a contract for my BES)
