cannot upgrade existing pushed app

Hi,
Am running BES 4.1.6, with a mix of BlackBerry devices (on OS 4.5 or above). I recently created a new software configuration containing two applications and their policies. I pushed these to the devices and everything installed fine.
One application needed to be locally configured so I went to all my devices and manually entered the settings. Not much I can do about that. So far so good.
However, an update for that application has been released. I have added the application to the application share and run loader.exe /index. It has been properly indexed and appears in the software configuration with the new version number.
However, my devices aren't getting the upgrade. I have tried reapplying the software configuration to users and clicking 'deploy applications'. This doesn't do anything.
I then creating a copy of that software configuration and applied that to users. The devices recognise a change has occured (presumably a new software configuration name, now called 'Copy of...') and prompt for a reboot. Upon reboot, nothing happens - the devices are still running the old version of the application!
The application has an updated application.xml file showing the later version number, so from what I've read, the devices should recognise the update and apply accordingly.
I don't know what else to try. I don't want to remove the application because I manually had to visit each device once to set the options and want to avoid doing that again. I was hoping an in-place upgrade would retain the settings??
Also, it's likely that another upgrade will be released within a few months and I'll have to go through this all over again!
Can anyone offer any advice or tips?
