If you want the carrier themes such as Orange, o2, Vodafone, T-Mobile , Etc... you can either edit your .alx file or download one of the modified .alx files jibi has posted in the ringtone/theme forum.
There are a few themes available from the mobile.blackberry.com site but afaik you need to download them via OTA and you can't use the desktop manager. The theme builder hasn't been released yet so there are no custom themes available right now. Once it's been released and people make custom themes for the Pearl you will be able to load them via desktop manager.
