If I use the slide out keypad, and press "o", I get the + and it works.
It's just that there is a + on the 0 key on the on-screen keypad, but it seems impossible to get it to work.
May not seem a big issue, but when you live within a few minutes of 3 countries you always use the prefix else you might not get through!

I completely understand, but I have yet to visit a country where 00 didn't work in a pinch, including places in the third-world. In fact, I've been to some countries where 00 was the only thing that worked.
Anyway, I checked a mate of mine's device and holding 0 gets a + on his 9800, so it seems to be an issue with your handset. Have you tried pulling the battery recently? Also have you updated your software recently?
