Before reinstalling the OS, try this from the HOME screen of your theme/BlackBerry:

Press the BlackBerry key
Press the BlackBerry key again
Select OPTIONS
Select THEME DEFAULTS RESET

This should remove any theme based settings (hiddin icons, icons that were deleted, folders you created, etc) and restore the theme to JUST FRESHLY INSTALLED state... and from there you should be able to find the wrench/options icon where ever it originally was.

And FWIW, if you have DIAL FROM HOME SCREEN turned OFF, you can get to the OPTIONS menu at any time by TAPPING the O key.
