On the BlackBerry phone go to Options>Security...

On the BlackBerry phone go to Options>Security Options>General Settings. Hit "menu" and choose "Wipe Handheld"

Further update: I did a Master Reset. I was then able to connect (finally) and perform a restore. Oddly, several applications did not restore so I am now reinstalling them.

Update: I've reinstalled BB Desktop Manager and USB drivers and I don't think it's anything at the desktop level. I've also tried to connect the upgraded Bold to another computer where no changes...
