You state that since "BES 5 HA requires that SQL be located on a separate physical server."

That is incorrect, I run SQL on my BES 5 box at my primary location with HA, I have another server at a remote location running BES 5 with SQL as the secondary. I have failed over at least 3 times now without incident.

SQL is setup as automatic failover with a witness server, although I can fail it over manually using SQL Server Manager Studio.

Also I am not running "Enterprise" SQL and it works fine..
