There isn't really many other things that could be wrong other than:
1. You don't have the right OS file.
2. You didn't delete the vendor.xml file.
Anyone else feel free to add to that list above.
Have you tried a different version of the Desktop Manager? I have yet to use the 4.5 version. I am still using whatever version T-Mobile has up for download on their website.
