Calendar Sync issue (ent 4.1)

Hi all,
I’m currently experiencing a few issues with Calendar syncing. If an appointment is put in Outlook it happily shows up on the Blackberry device, but if an appointment is placed in the Blackberry device it isn’t shown in Outlook.
I realise there are numerous posts regarding this issue, and issues like it, but trust me I’ve been through them and nothing seems to work.
I’m running the Blackberry Enterprise Server (4.1.4 MR5 159) and Exchange Server 2003 (SP2) on the same server (not ideal I know, but it comes down to cost!). These services are hosted on A Windows 2003 SP2 box with current updates.
A snippet from the event viewer shows the following:
Event ID: 20216 - {user@userswork.com} Synchronize() failed: ERR_FAIL, Tag=97796
The Enterprise Server MAGT log displays the following errors:

Although the Enterprise Server & Exchange are on the same box I’ve even resorted to re-registering the cdo.dll (C:\Program Files\Exchsrvr\bin\cdo.dl) just in case!
All permissions seem to be correct (BESAdmin send as, receive as, administer information store, Admin view rights etc.).
The strange thing is that all was working perfectly well just a week ago. There have been no alterations, no updates, nothing!
I’d appreciate any help, any suggestions, anything!
