What does that mean?

I want my BB to vibrate and sound at the same time when I receive a bbm.

It's cuz right now, when someone bbms me, it vibrates once, 2 seconds later vibrates again, 2 seconds later it sounds.

It means what you want it to do out of a holster, and what you want it to do in a holster.
A holster is a magnetised case, usually (but not always) comes with the BB.

You cannot (unless on OS 5 I think) have it vibe and sound at the same time.

Yeah, on device OS version 5.0, you can set the alerts to sound and vibrate at the same time.

I have OS 4.2 and it will let me do sound and vibrate together but it vibrates first and then rings, not both simultaneously.

Exactly. It vibes THEN rings.
OS 5 will vibe AND ring at the SAME TIME.
