After spending all afternnon trying to restore DBs (via DM) on my own, last night I finally "bit the bullet" and called AT&T. They picked up very quickly and the support person was very good. She tried many things I didn't think of (using DM to clear a few DBs), sending a reset from their server (with my phone off and battery out), etc. When they finally gave up, they suggested I do a Master Reset (wipe handheld). I was not ready to give up yet, so I asked to be transferred to BlackBerry. BlackBerry support did a few more tricks, without success. This even surprised them (go figure, why is it always me?). Eventually, even RIM gave up and he walked me thru a Wipe.
btw - if you're ever going to do this, it's a good idea to do a backup first.
I did:
- Backup via DM
- Wipe (this only took a minute or two, he said if I had encryption on it would have said scrubbing instead of wiping, and scrubbing would have taken hours since it resets every bit). Edit: There was a wipe option that allowed me to not wipe my 3rd Party Apps. I selected that. It was nice, the BB reset back to look like I just got it, but with all my apps there. I had to re-register them all since the system/common area where their keys were stored was wiped, but they were all still there!

- Restore only the Calendar and Address DB via DM
I'm still trying to remember all of my settings, but at least the icon is gone
