The "5000 additional messages" apply to messages that you send to US phone numbers while you are in your coverage area. It does not apply to to international phone numbers. And it does not apply when you are in another country.
If you send a BBM message to someone in another country, it will not cost you anything additional. It will be part of your data. Now depending upon what data package the other person has for their BlackBerry, it might cost them extra to send and receive BBM messages.
