Default Wired Calendar sync issues

Any help with this issue would be appreciated.

We have several individuals who perform a wired calendar sync with other individuals' calendars.
The sync is configured as a one way sync from Outlook to the BB.
The problem we are having is affecting multiple individuals.
At a certain point during the sync, the error "Unable to get an Outlook or MS Exchange record" comes up. Sometimes the error is more generic. Every once in a while it works, but that is rare and it isn't long before the error returns.
The RIM.log file shows "Internal Error #4238. Translation Canceled!"
We've gone through enabling advanced logging for Intellisync. Nothing in the tif.log or PrTrace.log files give any indication as to what is causing the error. (at least as far as I can tell)

I've gone through many of the steps to fix this that I've found here and in the KB articles from RIM. Nothing has worked.
New MAPI profile, deleting the Intellisync folder, checking permissions , not running Outlook in cached mode, tried different PC, tried different BB users syncing with the same problem calendar, clearing the Calendar on the BB, etc.

My questions are:
1. If the issue is caused by a corrupt calendar entry in Exchange, how do you find it, other than by process of elimination? Is there some hidden info in the tif or PrTrace log files?
2. I had one user give me permissions to see his private items, and now it works fine when I attempt to do the one-way sync with his calendar. Would that be the cause of this issue, not having access to see the private items on the other individual's calendar? Even if there are no private items present on said calendar?
3. Are 'reviewer' permissions (on the mailbox root and the calendar) sufficient for what we are doing? Would elevating the calendar permissions fix it?

Its almost safe to say its a permissions issue, but just to check post the tif.log if you can.
