If the BB is completely wiped out, it will have a funky icon on it with a code "507" - which means no operating system is found.
There is a sticky thread in the General Discussion forum about Version 4.0; it has the locations for every carrier on where to download the latest and greatest handheld OS. Find the right one, download it and execute it.
Then restart Desktop Manager - it should work.
