I believe in older versions, the BES only saw the first level folder of contacts. We used to instruct clients to have sub folders, for contacts not needed on the device. There was probably a complaint to RIM, and now the BES will push all the contacts that are seen under the contacts folder in Outlook (regardless of sub folder levels).

Inform your clients to place all contacts not wanted on the device, in a second folder under the root of the outlook mailbox. (or under any other folder than Contacts). The device will see the changes almost immediately if I recall correctly.

Note: You will also get asked this request if they pair, via bluetooth, their devices to their handsfree system in their cars (BMW, etc). People with many contacts take forever to sync with their car everytime, so using only Primary contacts in the Outlook Contact folder, speeds up the BlackBerry bluetooth sync process with their car.
