I am on T-mobile, NOT on BES, and I've tried all emulation modes and the error still occurs.

One is broken, out of the box, with wierd usb device errors. You alluded to a bad batch

Maybe that is why there is a VDC 5v connector on the back of the cradle? The power through the hub...

I've tried a few load a file to a web site services to do it like
